Touch Up Agreement *

I understand that my tattoo may not need a Touch-Up, and that determination of such is at the artist's discretion. Touch-Up determination depends on the photo(s) provided above and whether proper adherence to the aftercare instructions were practiced.

Touch Up Agreement Cont'd *

I understand that only tattoos done by Naomi within the last 6 months (from today's date) can receive a Complimentary Touch Up, and that fine line tattoos, micro tattoos and tattoos on the hand/fingers/elbow/foot/face/neck are excluded from this service.

No-Show and Last-Minute Reschedule Agreement *
